Controversies in Management of Patent Ductus Arterious in the Preterm Infant

چکیده

The management of patent ductus arteriosus in the preterm infant is one of the areas of clinical care that is subjected to great practice variation. This is sadly one of the consequences of widespread adoption of closure of the patent ductus arteriosus by pharmacological or surgical means without subjecting the treatment approaches to rigorous randomized control trials. The diverse approaches to treatment currently range from early and aggressive closure of the ductus arteriosus to a conservative approach of watchful waiting for spontaneous closure. This review reviews the complex management strategies of the ductus arteriosus highlighting the areas of greatest controversy that need to be addressed in future trials to provide greatest benefit to the vulnerable preterm infant. *Corresponding author: Bharati Sinha, Assistant professor in Pediatrics, Boston University School of Medicine, 771 Albany Street, Dowling 4 North, Boston, MA 02118, USA, Tel: 617-414-5519; Fax: 617-414-7297; E-mail: [email protected] Received July 20, 2012; Accepted November 08, 2013; Published November 11, 2013 Citation: Sinha B (2013) Controversies in Management of Patent Ductus Arterious in the Preterm Infant. Introduction: This study aimed to investigate CRP and platelet count changes in preterm neonates with Patent Ductus Arteriosus on the seventh day of birth compared to the third day. Methods: This longitudinal study was conducted on fifty neonates with PDA complications in Afzalipour hospital in 2017-2018. All neonates underwent repeated e...

Current controversies in the management of patent ductus arteriosus in preterm infants.

OBJECTIVE Patent ductus arteriosus is very commonly seen in very low birth weight (VLBW) infants, affecting about one-third. The present review tries to identify the group of VLBW infants who need active intervention in day-to-day practice and to determine the mode of intervention, based on current published literatures. METHODS We searched the Cochrane library, MEDLINE, EMBASE and CINAHL dat...

[Management of patent ductus arteriosus in preterm infants: An evidence-based approach].

A patent ductus arteriosus, very common in the preterm infant (incidence up to 70% in infants less than 28 weeks gestational age), causes a left-to-right shunt leading to overload of the pulmonary circulation and low systemic blood flow. These hemodynamic anomalies are associated with a higher incidence of respiratory, neurological, and digestive complications of prematurity, as well as a highe...

Patent ductus arteriosus in the preterm infant.

The ductus arteriosus is a communication between the main pulmonary trunk and the descending aorta. In utero, the ductus arteriosus allows blood flow to be diverted from the high-resistance pulmonary circulation to the descending aorta and the low-resistance placental bed. Functional closure of the ductus arteriosus occurs soon after birth but can be delayed in premature infants and in certain ...

Atrial natriuretic peptide and patent ductus arteriosus in preterm infants.

Preterm infants with symptomatic patent ductus arteriosus had considerably raised plasma concentrations of atrial natriuretic peptide. Surgical ligation of the patent ductus arteriosus was associated with an immediate fall in plasma atrial natriuretic peptide concentration. Thus left to right shunting and left atrial distension may cause atrial natriuretic peptide release in preterm infants wit...
